Type 2 Diabetes (T2D) Assessment

Freedom Diabetes Management recommends all clients with TYPE 2 DIABETES have an assessment every year. This assessment helps us to determine which types of support will deliver you the most benefit and value for money.

*This assessment is only for people with Type 2 Diabetes

Any client with Type 2 diabetes who has an assessment is then eligible for discounted individual consultations at Freedom Diabetes Management for the rest of the calendar year. Clients who have had the assessment completed elsewhere are also eligible for discounted services, but only if we are provided with the assessment report at the individual consultation i.e. it is provided along with the referral.

Clients who are suitable for group education will have exclusive access to our warm and inviting group education sessions throughout the year, starting May 2020.

Step 1) Download the ‘T2D assessment request’ from the ‘Resources’ page.

Step2) Show the first page to your GP to receive your referral.

Consider the sustainability options below.

  1. Email the document to your GP clinic (with your GPs name in the subject line).
  2. Email the document to yourself and show it to your GP on your smart phone.
  3. Print double sided if you prefer to complete the checklist and templates included on paper and take to your GP appointment.

Step 3) Start completing your checklist a week before your Freedom Diabetes Management consultation.

  1. Complete your checklist and templates electronically and email to ‘allirapanebianco@freedomdiabetesmanagement.com.au’.
  2. Complete your checklist on your notebook that you will bring along to your consultation.
  3. Print double sided if you prefer to complete the checklist and templates included on paper. Use free app ‘Cam Scanner’ on your phone to capture information on completed templates to make sure you have it with you at your appointment.

Step 4) Book your assessment appointment via our ‘Contact’ page.

Step 5) Take your referral and all the information you have gathered to your appointment to receive the most comprehensive and highly personalised care possible!

Group Education Sessions

Starting on the 9th of May 2020, Freedom Diabetes Management will start providing group education session. Only those who have been previously identified via assessment as suitable for group education will be invited to attend, along with a support person. Sessions will be delivered at Frankston South Community Centre and run for 2hours, from 9-12noon on four consecutive Saturday mornings and be followed by a delicious plant based lunch with an optional group walk to follow.

The introductory fee for each session is only $15 per person (support person pays a separate fee)-including a delicious plant based lunch.

EFTPOS facilities will be available.

Step 1) After your ‘T2D assessment’ you will be advised if our group education sessions will suit your learning needs or not.

Step 2) If our group education sessions suit your learning needs, you will be given more information about the courses of four sessions each that run on Saturday mornings. Courses will be held every second month starting May 2020.

Individual Allied Health Service

This service is for those who require individual attention to address their diabetes care needs.

Clients who have had a ‘T2D assessment’ completed prior can access discounted Individual Allied Health services for the rest of the calendar year! If required, a blood glucose meter can be provided to you free of charge at an individual consultation.

People who will benefit from Individual Allied Health service may have:

  • Metabolic syndrome
  • Pre diabetes
  • Type 2 diabetes
  • Gestational diabetes
  • Maturity onset diabetes of the young (MODY)
  • Latent autoimmune diabetes of adults
  • Monogenic diabetes
  • Type 1 diabetes
  • Preconception planning, pregnancy and follow up with Type 1 diabetes
  • Familial Hypercholesterolaemia
  • PolyCystic Ovarian Syndrome

During an Individual Allied Health Service you will have any specific challenges or education needs addressed.

Learning or support needs may include:

  • What is diabetes (or related health condition)
  • Annual review
  • Newly diagnosed
  • New medications
  • Monitoring and interpreting blood glucose levels
  • Using Freestyle Libre flash glucose monitoring or Continuous Glucose Monitoring (CGM)
  • Insulin pump therapy
  • Diabetes phone apps
  • Diabetes complication management
  • Diabetes and sexual health
  • Diabetes and mental and emotional health
  • Stress management
  • Diabetes nutrition
  • Physical activity and diabetes
